FUTSOC Tryout Waiver, Release of Liability & Emergency Authorization
I, the undersigned parent or legal guardian of the participant listed above, hereby authorize FUTSOC, its directors, coaches, trainers, staff members, volunteers, representatives, and designated emergency contacts to act on my behalf in the event of an emergency and to consent to any necessary emergency medical, surgical, dental, or hospital care and treatment for the participant if I cannot be reached immediately.
I understand and acknowledge that participation in soccer tryouts, training sessions, practices, scrimmages, conditioning activities, transportation, and related activities involves inherent risks of injury and danger. These risks include, but are not limited to, falls, collisions, contact with other players or equipment, adverse field conditions, heat-related illness, communicable illnesses, negligent acts of others, and serious physical injury, including bruises, sprains, strains, torn muscles or ligaments, fractures, concussions, traumatic brain injuries, spinal injuries, paralysis, and death.
I voluntarily and knowingly accept and assume all such risks on behalf of myself and the participant.
I certify that the participant is physically fit and able to safely participate in FUTSOC tryout activities and that I have not been advised otherwise by a qualified medical professional.
In consideration for allowing the participant to participate in any FUTSOC tryout, training session, practice, scrimmage, evaluation, transportation, or related activity, I, on behalf of myself, the participant, our heirs, assigns, representatives, and next of kin, hereby release, waive, discharge, and agree to hold harmless FUTSOC, its owners, directors, coaches, trainers, staff, volunteers, representatives, sponsors, affiliated organizations, and any parks, schools, facilities, municipalities, or property owners utilized for activities, including but not limited to the City of Weston, Broward County, Tequesta Trace Park, Heron Park, and any other utilized facilities, from any and all liability, claims, demands, actions, damages, costs, or expenses arising out of or related to any injury, illness, property damage, loss, or death resulting from participation in FUTSOC activities, including claims arising from the ordinary negligence of any released party.
I further understand and acknowledge that transportation to and from activities or emergency medical facilities may involve additional risks, and I voluntarily assume those risks.
I grant permission to FUTSOC to use photographs, video recordings, or other media containing the participant’s image or likeness for promotional, marketing, social media, or educational purposes without compensation or further approval, unless otherwise prohibited in writing.
I understand that this agreement is intended to be as broad and inclusive as permitted under the laws of the State of Florida. If any portion of this waiver is determined to be invalid, the remaining portions shall remain in full force and effect.
By checking the required waiver agreement box and submitting this form, I acknowledge that I have carefully read and fully understand this Waiver, Release of Liability, and Emergency Authorization. I understand that by submitting this form, I am giving up substantial legal rights on behalf of myself and the participant, including the right to sue, and I agree to these terms freely and voluntarily without inducement.
Tryout participation does not guarantee placement on a FUTSOC team.
